Has your Health Insurance Card
been lost, stolen or damaged? No problem. You can replace it online.
But first, make sure you have a Government Authentication Service account.
And if you’ve moved recently, double check that you changed
your address, so your card is delivered to the right place.
Then, click on “Apply online”. Identify yourself using the
Government Authentication Service. Once you’re logged in, enter
the required information. Select the person who needs to replace
their card and the reason for replacement. Finalize your transaction and
voilà, your request is sent!
